Essential Duties:

  • Responsible for the Clinical activities and Clinical staff. Provide effective oversight and communication, working closely with another Clinical Nursing Supervisor, to ensure accurate, timely delivery of appropriate clinical services to care for the needs of individual injured workers; and to secure a timely return to work status.
  • Support the Mission, Vision, and Values of HomeCare Connect, promoting positive public relations. Ensure that clinical staff maintain a high level of ethical and professional standards, continuously improving healthcare services. Provide optimal leadership and resource support to staff, serving patients with high quality, cost effective care. Participate in Q.A. and departmental improvement efforts. Provide appropriate follow-up and updates.
  • Train, coach, and lead clinical staff utilizing appropriate tools and resources to properly perform the case management process. Plan and organize individual caseload to meet regulatory and Best Practice Standards. Identify any concerns within the department and/or quality insurance and risk management issues.
  • Appropriately research, review, analyze and prepare medical information, document pertinent information in the case file notes, apply evidence based established guidelines to support interventions and facilitate the case management process.
  • Assist with on-call duties on assigned weekends and is compensated accordingly.

Job Requirements:

  • Current RN or LPN state licensure in good standing with the State Board of Nursing.
  • Previous Work Comp Case Management experience with at least three (3) years hospital/clinical experience in Orthopedics, Neurology, Occupational Health, Med/Surg or other comparable clinical discipline.
  • Significant knowledgeable of injured worker's rights and ability to ensure an atmosphere which to promote the privacy, dignity, and well-being of injured workers.
  • Skills: Excellent leadership, verbal and written communication, team building, customer service, and conflict resolution skills; Promptly address concerns with tac and professionalism; proficient reading, writing, grammar, and mathematics skills; strong medical terminology; Excellent organizational, prioritization, and computer skills (e.g., Outlook, Teams, to be trained on proprietary software).
