Home Health Coordinator

HomeCare Connect, LLC, located in sunny Winter Park, FL seeks an experienced Home Health Coordinator for its fantastic Home Health Team. Responsible to provide appropriate staffing for home health referrals, serve as point of contact between Providers and Injured Workers, and identify Network Providers to ensure reliability, professionalism, and responsiveness. Ensure positive interaction with and addresses questions and concerns from Adjusters, Case Managers, Providers, and Injured Workers in a timely, professional manner. Preferred: Some college and/or medical background. LPN preferred. At least one (1) year of related experience working with Provider Relations (e.g., healthcare, DME, insurance, physician practice).

General Responsibilities:

  • Provide staffing for home health and IV therapy referrals, assist Intake Department with taking new home health referrals as needed. Serve as point of contact between Providers and Injured Workers. Promote positive public relations with referral sources, and identify Network providers to staff and accept home health referrals. Address inquiries and questions in a timely, professional manner. Provide excellent follow up as appropriate with supervisor, coworkers, clients, and other internal and external partners.
  • Answer phone calls, document management, intake referrals, letters of authorization, and pricing negotiations. Demonstrate appropriate knowledge of Injured Workers' rights, ensure privacy, dignity, and well being of Injured Workers.
  • Support the mission, values, and vision of Home Care Connect through effective communication with internal and external customers, serve as positive role model, demonstrate excellent customer service, and comply with all applicable regulatory requirements and Quality Assurance initiatives.

Requirements:

  • Previous healthcare experience, preferably Home Health. Experience with Referrals and Authorizations.
  • Computer Proficiency, Medical Software, MS Office, and Outlook.
  • Excellent customer service, verbal and written communication skills.
  • Ability to multi-task and prioritize.
